Request Form

The deadline to request a hard-copy check was June 23, 2025. If you requested for a hard-copy check and completed a W-9 form before the deadline, then your check will be issued and mailed to you on or before July 15, 2025.

If you did not timely request to receive a hard-copy check, then the second payment will be sent to your Stripe account on or before June 30, 2025.
